You may choose to remain anonymous.
You do not need to include your name or any identifying information unless you would like follow‑up.

  • Receive services without your feedback affecting eligibility or support

Confidentiality

All submissions are reviewed only by STHS leadership.
If you choose to remain anonymous, we will not know who you are and will only be able to act on the information shared.
