Outreach
Our outreach worker provides direct services to clients including needs assessment, information and referrals, safety planning and local transportation assistance.
The worker also acts as a client advocate and accompaniment when appropriate and possible for medical, legal or social service appointments.
They can also assist clients in setting up appointments and providing transportation when necessary to clients to access resources relating to service goals.
All services are provided using a client centered individualized approach.
Privacy and anonymity are always a priority.
All clients will be served in a dignified, respectful, transparent, non-judgmental, strengths based, holistic manner.
